The following paperwork is needed in order to participate in MOSSL: (click on #1 + #2 to download from MOSSL)
1.  MOSSL Player Registration Form (USYSA FORM)
2.  Ohio South (OSYSA) Medical Release Form
3.  Picture (driver's license size)
4.  COPY of Birth Certificate (not original)

(*Coaches- need Team Roster Form, Risk management online, Proof of License, and  card w/picture and signed)

DIRECTIONS- (to practice facilities)

1. (old) Jackson High School-  (baseball field,track)

2. Mt. Logan Middle School Field (Chillicothe)- located on Main St., from Rt.35- take Main St. West (toward town), turn Right at 2nd light.

3. Barthelmas Park (Circleville)- located on Kingston Pike,
from SOUTH on Rt.23- turn Right at Tarlton Rd. (traffic light, Captain D's), turn LEFT at 4-way stop sign onto Kingston Pk., park is approx. 1 mile on right side.

from NORTH on Rt.23- turn LEFT at Tarlton Rd. (traffic light, KFC), turn LEFT at 4-way stop sign onto Kingston Pk., park is approx. 1 mile on right side.

from WEST- take Rt. 22/Main St. through downtown, past UDF, turn RIGHT at traffic light (Speedy Muffler), park is approx. 1/2 mile on left.
